What is ECMO?

ECMO means extracorporial membrane oxidation. The ECMO machine is similar to the heart-bypass machine used for open-mortar surgery. When your child is placed on the ECMO, his blood receives oxygen from an artificial lung in the ECMO circuit (or system). Artificial lungs in the ECMO circuit will provide your baby's blood to live with oxygen, until its lungs and/or hearts are not able to work on their own. When ECMO is used:

  • For patients recovering from heart failure, or lung failure or heart surgery.
  • As a bridge option for further treatment, when doctors want to assess the condition of other organs such as kidney or brain before performing heart or lung surgery.
  • For support during high -risk processes in the cardiac catheterization lab.
  • As a bridge for a heart assist device, such as Ventricular Assist device (LVAD).
  • As a bridge for patients waiting for lung transplantation. ECMO helps to keep tissues well -rich, which makes the patient a better candidate for transplantation.

Risk

    ECMO does carry risks including:

  • Bleeding, due to the drug which is given to prevent blood from clots in tubing.
  • Infections on sites where tubes enter the body.
  • Inaccurate problems, as blood products are given to a person on ECMO.
  • Tubing make small clots or air bubbles.
  • The possibility of stroke increased.

Cardiac ECMO

In children with very poor heart function, ECMO can serve the child's heart. It provides time to relax and fix the heart, while maintaining a good blood supply to the brain and other organs in the body. Cardiac ECMO may be required after open heart surgery, when the heart may swell, may be unable to maintain high enough blood pressure or have an irregular rhythm. It may also be required due to infection affecting the heart muscle (myocarditis) or heart muscle failure (myopathy), where the heart cannot effectively pump blood around the body. Please note that ECMO can only help children whose lungs and/or heart disease are reversible within about three weeks.

Types ECMO

There are two types of ECMO, VA (Voo-arterial) and VV (ven-venous):

Va ECMO : Two penetrations (tubes) are placed in your baby's blood vessels, one in one vein and the other in a artery, usually on the edge of the neck. Dark blood (slightly oxygen -rich) is constantly dried in the ECMO circuit from the tube in the vein, while the ECMO circuit is now returning the body through the tube in the blood artery containing oxygen.

This type of ECMO provides support for both heart and lungs and therefore requires cardiac or respiratory ECMO support for children. For a low number of children required by ECMO after open heart surgery, canula (tubes) can be inserted directly into the heart through the chest during the operation of the heart instead of the neck vessels.

VV ECMO : A single catheter is placed in a vein, usually in the edge of the neck. The blood is extracted from this catheter at the same time in the ECMO circuit as the blood-rich blood is returned from the ECMO circuit to the child through the same catheter. VV ECMO only provides lung support and does not support the heart.

Some children who start with VV ECMO will need to be converted into VA ECMO if their heart also needs support.

How does ECMO Work?

ECMO can be started in the operating theater immediately after surgery or on one of the intensive care units. If you are going to ecmo after a heart Operation, surgeons will usually enter the tube (tube) during the operation directly through the chest.

If the ECMO is initiated in the intensive care unit, the penetration connecting the patient to the ECMO circuit is placed directly into the blood vessels on the side of the neck. This small operation occurs on the bedside in the intensive care unit and will be asked to leave during the process. A mild anesthetic is given, so your child does not feel any pain and a tube (penetration) is placed in a large vein on the side of the neck. A second tube is required, which is also inserted into the artery on the side of the neck. Doctor For ECMO Treatment In Palanpur, Gujarat

Once in place, Entrance is then associated with the ECMO circuit and machine. Through the tube in the vein, your baby is pumped through dark blood (slight oxygen) drains and membranes oxygen (an artificial lung) where carbon dioxide is removed and oxygen is added. The blood is re -heated and returned to the body. This process runs continuously while your child is on ECMO. They will also stay on a ventilator, but on very soft settings that allow the lungs to relax.
